Consent & Post Exit Form: Designs

Below you will find examples of a Consent Form and a Post Installation Form designed in accordance with KCL requirements & regulations. The forms were filled out by every single person who participated, in other words willingly chose to experience this installation.

  • The Consent Form was a measure taken to fulfil the KCL Ethics Requirement for human research, and to keep a precise track of the participants’ data, as well as to ensure that each person has fully understood the nature of the participation and has willingly consented to be a part of my research project.
  • The Post Installation Form enabled the researcher (myself) to gain access to a pool of candidates who consented to carry out the questionnaire and interviewing stages. Each person provided the necessary information (please refer to the PDF) for gaining an initial understanding of which participants would be suitable for further research.
